Job Description
Identify and assess the nutrition needs of patients and residents. Initiate interventions to achieve and maintain his/her nutritional status. Provide education to patients, residents, staff, and the public. Act as a resource to the department and the facility.

KEY RESULT AREAS/ESSENTIAL FUNCTIONS OF THE JOB
  1. Participates in menu planning following guidelines for each facilities license and manages the menu management software.
  2. Assist in daily operations of the department and may have responsibility of the department operations in the absence of the Culinary Services Director.
  3. Performs initial assessment on appropriate patients, demonstrates working knowledge of illness, injuries, and diseases using commonly known resources in the assessment process and reassessing as per policy.
  4. Documents the results of patient care within established policies, procedures, and scope of practice while consistently maintaining accurate charts and data and documenting within established time frames using approved terms and abbreviations, following HIPAA privacy guidelines.
  5. Develops the plan of care based on patients’/residents’ psychosocial, cultural, ethnic, and religious/spiritual needs, develops, evaluates and revises plan of care as indicated by the patient’s response to treatment, patient interview, and discussion with family. Works with the health team to ensure the highest patient care.
  6. Plans and provides patient, family, and caregiver education and identifies learning needs by clearly communicating information and documenting patient/resident response to education.
  7. Plans and provides community education, works with support groups, identifies learning needs, clearly communicates information, and solicits feedback on presentations.
  8. Work collaboratively with the care provider teams.
  9. Promotes the Benedictine Mission and Core Values of Hospitality, Stewardship, Respect and Justice by bringing the Mission and Core Values into the day-to-day activities of the company.
  10. Performs other duties, tasks and/or projects as assigned.
      

Required Experience: 

  • Ability to read, write and communicate in English.
    Required to analyze data, remember and recall multiple policies and procedures.
    Must have organizational skills, ability to assess, plan, implement, and evaluate patient care.
    Must be able to demonstrate multiple priorities and the ability to direct the activities of others.

Required Education:
